Please complete and return this form to Children’s Discovery Museum a minimum of 24 hours prior to your event. In case of last minute changes to your guest list, please update this form and give it to our on-site staff member.
Due to current COVID-19 requirements, it is important that the above information is accurate. This information will be kept on file and, should the need arise, made available to the Santa Clara County Health Department for contact tracing purposes. Thank you!
